Description

Required to work within a 0-19yrs Childrens Service, responding to the needs of the local community, with regard to Health, learning, play and parental support, assisting in the delivery of a family centred Public Health Service to a defined population.

Working with a multi-disciplinary agency approach.

Identify needs in children, young people and their families who have emerging drug and alcohol needs and require further specialist referrals.

Ensure that the work is underpinned by a community development approach to meet the local health needs.


To contribute to, and advise on, the development and implementation of service provision which meets the local health needs and targets.

In particular, to facilitate parental involvement in health-related learning and development.

To participate in the development of a range of health promotion initiatives which aim towards improving the physical, social and emotional well-being of children and young people.

To work under the direction of Specialist Health Visitor and specialist nurse for complex and vulnerable families and contribute to the delivery of the service and deliver the Healthy Child Programme.

To contribute to the assessment of need with children, young people, their families and communities. To participate in planning, implementing and evaluating interventions and support, contributing to health and well-being. To lead on Early Help Plans, complete Early Help Assessments and Plans.
