About the Role:

Billericay PCN has an opportunity for 2 care co-ordinators to join our primary care network (PCN) on a full time basis, to support our GP practices to assist in re-shaping primary care across our communities.

You will be based at one site in Billericay albeit there may be a need to travel across practices in the locality if required.

You will enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve people's quality of life and play a key role in proactively co-ordinating primary care patients to ensure they are provided with the most appropriate service for their needs.

You will work closely with all primary care staff, to ensure:

appropriate information is provided to patients.

- support is made available to patients as and when appropriate referral pathways are undertaken.
- administrative duties in the management of day-to-day operations within primary care are undertaken.
- attend multi-disciplinary meetings as required.

Excellent interpersonal and communication skills.
Organised, patient and empathetic.
Experience of working in health, social care or other support roles including direct contact with people, families or carers.
Must be proficient in the use of Microsoft packages.
