An exciting opportunity has arisen for 5 dynamic individuals to join our admissions team as a Planned Care Admissions Navigator.
The post holders will be responsible for the efficient management of specific waiting lists through from the date placed on waiting list, to day of operation, in compliance with local and national performance targets.
This is an excellent opportunity to develop existing healthcare administrative skills in a demanding and varied, but rewarding role in a busy and challenging environment and we require a proactive and confident person to fill the post.
The post holders will liaise with the multi-disciplinary team, internal departments and external organisations to ensure continuity in achieving excellence in patient care.
The post holders will work as part of the Planned Care Admissions team, booking admission dates and arranging multiple investigative appointments for the Planned Care Admissions service.
To adhere to Trust and National waiting list targets and policy.
To support Assistant Service Managerclinical staff (as required) in prioritising elective admissions.
To be responsible for accuracy of waiting lists/activity and to action/highlight any errors to the Assistant Service Manager

To be responsible for managing patients, waiting lists and admissions for a number of
To fully involve the multidisciplinary team in the planning of admissions, ensuring that the patient is provided with appropriate information regarding their admission in a timely manner.
To ensure that patients are booked into the appropriate pre assessment appointment service, and also booked to see a Consultant Anaesthetist where appropriate for highly complex patients.
To construct and schedule theatre lists as per department / specialty guidelines.
To ensure any patient cancelled on the day of admission/surgery for non-medical reasons, is reallocated a date for admission within 28 days, escalating any problems or issues within a timely manner to the Assistant Service Manager.
To manage waiting lists activity and admission data [i.e. PiMs] and other associate IT systems [i.e. Excel, PAC's, EPR, Galaxy, PTL] and action/highlight any
To ensure all patients have stopped any medication as advised by the pre-assessment team prior to surgery.
Maintain thorough and accurate records of all correspondences with patients throughout their admission process, including updating of the PTL system and EPR as appropriate
Update PIMS, PTL, and Galaxy and enter details on the Planned Care Admissions Specialty lists after directly agreeing an admission date with the patient to ensure accurate records and to comply with trust guidelines for updating theatre lists in a timely fashion.
Ensure as far as possible that operating theatre capacity is fully utilised by way of allocating patients to lists in accordance with department / specialty guidelines, clinical resources and consultant clinical requirements.
Liaise with consultants to ensure theatre lists are booked according the clinical priority and weeks wait according to the national waiting time expectations.
