At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier It's a super exciting time to join us as we begin to deliver the second iteration of our Integrated Health and Wellbeing Service (IHWS) in East Sussex, and we're looking for a Primary Care Liaison Officer to join us

Our programmes are designed to support individuals to improve their health behaviours and make sustainable changes to their lifestyle. The IHWS programme in East Sussex consists of five key services;

  • Stop smoking support
  • Physical activity programmes
  • Adult weight management
  • NHS Health Checks
  • Health Coaching


Residents across East Sussex will be able to benefit from a variety of our established pathways and products, including Gloji and MAN v FAT Football.

The Primary Care Liaison Officer aims to provide a dedicated point of contact in the organisation and an effective link between primary and secondary care.

Our focus is to forge strong relationships with primary care colleagues and continually explore ways to build and improve these relationships to ensure the best care.

A typical day as an Primary Care Liaison Officer might include:

  • Extensively working with our primary care colleagues to assist the delivery of all OYES KPI's with a key focus on stop smoking
  • Working closely with primary care partners on two key primary objectives (Referrals into the OYES stop smoking service and the recoding of current patient records where possible).
  • Working towards a full OYES SMART Card coverage within every surgery within East Sussex.
  • Working with local surgeries and PCN's to ensure the whole OYES service is promoted and marketed to a high standard
  • Working with local PPG groups to promote the wider OYES service
  • Ensuring GPs and practice teams are kept up to date with relevant news
  • Build extensive relationships between primary care and OYES
  • Arrange Practice visits and events
  • Making improvements in communications and services and creating opportunities for clinicians to work together for better patient care
  • Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments we're always looking to share knowledge and resources
  • Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

Requirements:


  • Experience with working with colleagues within Primary Care
  • Good overall knowledge of healthy lifestyle services and outcomes
  • An ability to communicate effectively and work closely with staff in a range of settings, particularly GP Practices.
  • Excellent time management skills with the ability to work on a number of tasks simultaneously
  • Quality focused, with knowledge or experience in the implementation of clinical governance or quality assurance frameworks in a service delivery environment.
