The SEN team complete the Education Health and Care (EHC) plan needs assessment process in line with the 'Children and families act 2014', and the 'Special educational needs and disabilities (SEND) 0-25 years code of practice'.

We review the EHC plan on an annual basis.

We provide schools with advice and guidance on the procedures adopted, statutory rights of appeal, admission arrangements; and any other issues arising out of the assessment of a child's needs.

We provide parents with a key point of contact opportunities to discuss progress and provision, details of their rights of appeal; signposting to other appropriate services and more.

Our SEN Caseworkers work closely with stakeholders (parents, schools, Health, Social Care and other disability professionals) to ensure that children and young people with SEN and disabilities have appropriate EHC Plans.

They read and dissect information following Annual Reviews to feed into amendments using SEN knowledge to write up new Outcomes and Provision as required.

They write parts A and B of EHC Plans for Officers using information gathered during the assessment process - pulling important details and writing relevant and useful sections for the EHC Plan.

They co-ordinate phase transfer processes under the guidance of SEN Officers (children and young people transitioning from Nursery to Reception, Year 2 to 3, Year 6 to 7 and Year 11 to Post 16 education).

They contribute to the monitoring and evaluation of the service. They contribute to corporate project-based activities and provide flexible professional input to national and local initiatives as required.

The Team works closely with the young people and their families so a family friendly approach to working practice along with a confidence and commitment to making legally sound decisions on behalf of the Local Authority is essential.

  • We are committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expect all staff to share this commitment. Applicants to posts that are exempt from the Rehabilitation of Offenders Act will require a DBS from the Disclosure and Barring Service before the appointment is confirmed._
