Job summary

You'll play a key role in delivering safe, patient-centred urgent care directly in people's homes, ensuring the highest standards of care during out-of-hours service.

As a valued member of our clinical and operational team, you'll work independently, using your expertise to assess, treat, and support patients with a wide range of conditions.

You'll provide both in-person care in the community and telephone advice, ensuring patients receive safe, effective, and timely support.

Responsibilities

• Deliver holistic care in patients' homes, from assessment through to treatment and medication administration.
• Provide safe and effective health assessments and telephone advice.
• Work collaboratively as part of a multi-disciplinary team, ensuring seamless, evidence-based patient care.
• Maintain accurate and timely records in line with NMC guidelines.
• Support and supervise junior team members, sharing expertise and promoting best practice.
