Are you seeking a new challenge in 2023, if so, this may be the new and exciting role you've been looking for The Team is responsible for the case management for people with complex health needs.

An opportunity has arisen for us to recruit 4 Case Management, Team Leaders.

As team leader you will be responsible for the management of the locality case managers and will hold a caseload of cases with complex ongoing health conditions.

You will get to know your patients and the cases of the patients that you case managers work with and develop a long-term relationship with them, their families and care provider to make a positive difference to the experiences to those that your team cares for.

You will be responsible for ensuring that their needs are met and regularly reviewed in line with local and national requirements and undertake eligibility assessments as indicated.
