We have a great and exciting opportunity for a new Keyworker service within our Children and Young people's service.
The Keyworker service is part of the NHS long term plan to support with a joined up working and consistent approach for each child and young person with a learning disability and/or children and young people who are Autistic with the aim to help to reduce children and young people in crisis and to prevent admission or re-admission to hospital, helping them to access the right help at the right time across complex systems.
You will be supporting and implementing change for children and young people with a learning disability and/or children and young people who are Autistic or both, you will be committed to building trusting relationships with children, young people and their families and passionate about making a real difference.
Key working service will aim to ensure that children, young people and their families get the help they need to navigate complex systems and are supported by someone that they have built a trusting rapport with.
Key Responsibilities
- You will be supporting children and young people with a learning disability and/or children and young people who are Autistic through transition between services, through key stages in their lives and provide a single point of support to help make the connections between children; young people; families; carers and the array of services & support available.
- To identify with the family the most important areas where they would benefit from additional support and work with them intensively to enable positive change, address any concerns, ensure gaps in services are challenged, escalated and acted on so to reduce risks and achieve the agreed outcomes.
- To advocate for children and young people and their families and the ability to challenge others, safeguarding, problem solving and supporting with solutions and that they are supported to be recognised, validated and empowered to live the lives they want.
- As a genuine ambassador and role model for our organisation, you will bring our values to life and share our passion and commitment to always strive to find creative ways in making a difference.
As a Keyworker you will play a pivotal role to ensure child-centered and a whole family approach, to meet their needs, aspirations and outcomes for the children and young people.
You will need to have experience of working with children, young people with a learning disability and/or children and young people who are Autistic and their families.
A comprehensive understanding of the needs of people with learning disability and/or children and young people who are Autistic and their families as well as safeguarding.
This is a Fixed Term Contract until November 2024, with possible extension.
