We are looking for a band 3 'Dementia Support worker', who has an understanding and/or experience in working with clients diagnosed with Mild Cognitive Impairment or Dementia, and the carers who support them.

The successful applicant will join a small team of dementia support workers who form our Dementia Support Service (DSS). This service is provided by LPFT on behalf of Lincolnshire County Council. This is a 37.5 hour post to predominantly cover Spalding and Stamford areas.

The core hours of the service are 09:00 - 17:00 Monday to Friday where possible, although this may be negotiable.


The postholder will be required to undertake appointments face to face in people's homes, by telephone or digital media (such as teams) and as such will be working in a hybrid model of remotely from home and in the workplace.


As a dementia support worker, you will support and enable our clients and carers to engage in assessment, to create a personalised support plan and subsequent review discussions.

The postholder will also offer support and guidance to the client about specific topics such as living well with dementia, information appropriate to their diagnosis and individual needs, signposting to services which may be able to offer specific or ongoing support and advice.

The dementia support workers are aligned to our community mental health teams and as you will need to be able to work with CMHT colleuge's


The Dementia support workers contact patients ( post diagnosis) and or carers to arrange a review of their needs in relation to understanding the diagnosis, living well with dementia, sign posting to agencies that can support with financial issues and lasting power of attorney.

The post holder will build up local knowledge to have an awareness of what is available for people to access in the local community.


People with dementia and carers reviewed by this service need to have a personal support plan written up and agreed.

This plan is created by the people that access our service an your role will be to give them the knowledge or links that they identify as meaningful to them.


We aim to support as many people post diagnosis as possible and one of our key performance measures is that we co create the support plan within 4 weeks of receiving the referral and that we review and discharge at the maximum of 12 weeks post referral.


An ability to work alone and on MS teams is essential as is the ability to work within the CMHT environment so that you are able to be part of our memory assessment pathway.

The successful applicant must have excellent communication and listening skills, be able to be a team player even when working remotely and to be able to type and input information into our clinical system.
