Lavant Road Surgery are looking to employ a Care Coordinator Care coordinator to work alongside and in partnership with our existing care coordinator team.


Care Coordinators provide extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals.

They will work closely with the GPs, and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.

They focus delivery of the comprehensive model to reflect local priorities, health inequalities or population health management risk stratification. This is a developmental role and therefore requires a flexible approach and an ability to accept a changing remit.


Key role requirements

Care coordinators will:

  • Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.
  • Chase non attenders for baby imms promoting the benefits.
  • Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.
  • Support the recall of patients with long term conditions who require appointments.
  • Liaise with patients with additional educational needs and their carers to arrange appointments for their annual health action plan.

Training:


  • To be determined by the needs of the post holder, in conjunction with the requirements of the role. Training will be provided and funded.

Communication:


  • Effectively communicate at all levels of the organisation; to a variety of health professionals; users and carers and external stakeholders to provide the best outcomes.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures & in line with NMC guidelines.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
  • Utilise and demonstrate sensitive communication styles, to ensure patients, their carers or those responsible for the patient, are fully informed and consent to treatment and care management;
  • Where necessary, employ alternative methods to overcome different levels of understanding, cultural background, and preferred ways of communicating;
  • Attend and contribute to practice and primary care team meetings.

Key areas of Knowledge

  • To have a detailed knowledge of priorities for local services, and to understand the resources available.
  • To use multiple IT tools to review and update patient data and to ensure that any updated data is accurate and factual.
  • Proven ability to recognise and manage risk.
  • Knowledge of local care pathways

Special Working Conditions:


  • May at times be a lone worker;
  • Required to potentially travel independently between patients and to attend meetings;

Qualifications:

GCSE English and Maths or equivalent and NVQ level 2 or equivalent


Desirable:


Experience:

Minimum experience of 3 years working within General Practice (A)

Previous experience in reception or administrative role with customer contact and use of electronic records and databases (I) (A)
