An exciting opportunity has arisen for a full time permanent post as a Discharge Coordinator on Ward F19.

Ward F19 is a 28 bed Trauma and Orthopaedic ward. Our patients may require further community input from varied specialist teams due to the nature of their injury. These teams can include social care and therapy services.


This role requires staff to work closely with the multi-professional team on Ward F19, Trauma and Orthopaedics and the wider NUH team.


The aim of the role is to ensure that all patients are tracked through their ward stay, to ensure timely discharge including highlighting any escalations where necessary.


The post holder will have awareness of and know how escalate delays along a patient's journey, working with the ward team, Integrated Discharge Team and Bronze on Call as required to minimise these.

The post holder will support the Nurse in Charge and bay nurses to complete any non-clinical tasks which may expedite the patient journey, thereby releasing nursing staff to better support the ward and care for their patients.


Delays to work through will include waits for diagnostic scan, wait for clinical review of reported scan, wait for completion of TTO, and wait for discharge following TTO.

Actions the post holder will implement to reduce delays will include liaising directly with the clinical staff on the ward, liaising with diagnostic specialties, booking transport, and speaking with patients and relatives.


The post holder will work proactively, pre-empting delays where possible, and helping to plan for discharge when a patient is 'maybe-home'.

F18, F19 and F20 and FADU all work together as a unit and there are 3 discharge co-ordinators across the unit.


This person is expected to work in the ward environment, alongside the multidisciplinary team to ensure that effective discharge planning and communication are managed in order to provide a smooth, timely discharge from hospital.


The discharge coordinator will be working alongside the large multi-disciplinary team of nurses, doctors, physiotherapists, care support workers, occupational therapists and social workers.

The Discharge Coordinator would be expected to liaise with the Integrated Discharge Team following the Discharge to Assess pathways as well as working with the nursing and medical staff to ensure prompt discharges from the ward via the Discharge Lounge as much as possible.

They would also be expected to help the team keep the ward environment clean and tidy and be an approachable helpful member of the ward team.

Other responsibilities include answering phones, liaising with patients and their relatives and chasing any delays to facilitate a safe discharge.
