SECONDMENT OPPORTUNITY - ROUGH SLEEPERS MENTAL HEALTH SERVICE (WALTHAM FOREST)
An exciting secondment opportunity has arisen for a B6 Social Worker, OT or CPN to join the NELFT Rough Sleepers Mental Health Team.
This new service became operational from March 2020 supporting Rough Sleepers with mental health difficulties who may have difficulties engaging with mental health services through the usual pathways.
The team works across the four NELFT boroughs, and a vacancy has arisen for the practitioner based in the Waltham Forest area.
The teams operational working hours is Monday to Friday 9am to 5pm although some flexibility may be needed, a car driver would be desirable but not essential for the role.
The main responsibilities of the post:
To provide an integrated recovery centred clinical service to people aged 14-65 working with rough sleepers or homelessness, and have the skills to assess the housing needs of clients with mental health difficulties.
To access and act as care coordinator to clients referred to the service, in accordance with Care programme Approach (CPA) policy.
Undertake comprehensive assessment of health and social needs together with the service users implement a plan of care and intervention to meet these needs, ensuring regular reviews are in place for evaluation of the care plan.
Within the CPA process monitor and assess the physical health of service users with mental illness in line with Trust policy and national guidelines
To facilitate and implement the Trust and Local Authority requirements in relation to safeguarding children and guidance legislation within the Care Act 2014.

Probationary Period
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.
All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction.

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177).
