This is an exciting time to join the Cardiac Rehab team within the Cardiology Department as we expand the services offered.


We are looking for a friendly, enthusiastic and innovative individual who will work well within the team to assist in the delivery of a comprehensive programme across various locations including community-based sites.


  • As part of a multidisciplinary team, assist in the delivery and development of a comprehensive cardiac rehabilitation programme.
  • To provide exercise therapy tailored to the needs of a patient referred to the MCHT Cardiac Rehabilitation Service.
  • Using evidence based principles/current best practice, specialist skills and clinical reasoning to assess, plan, implement and evaluate interventions and outcomes.
  • Managing own patient case load, including preparation and completion of patient assessments to establish physical, psychological and educational needs and goal setting plans including families and carers and being responsive to any communication barriers.


Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond.


The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.


We employ almost 5,000 members of staff, have 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health.


The Trust is also responsible for the provision of a range of community services for people across South Cheshire and Vale Royal, working in partnership with local GPs and Cheshire and Wirral Partnership NHS Foundation Trust.

(MCHFT) is looking to ensure that we provide equity of services across seven days of the week. This post may be reviewed in line with this plan and in some cases an element of weekend working may be required.

  • As part of a multidisciplinary team, assist in the delivery and development of a comprehensive cardiac rehabilitation programme.
  • To provide exercise therapy tailored to the needs of a patient referred to the MCHT Cardiac Rehabilitation Service.
  • Using evidence based principles/current best practice, specialist skills and clinical reasoning to assess, plan, implement and evaluate interventions and outcomes.
  • Managing own patient case load, including preparation and completion of patient assessments to establish physical, psychological and educational needs and goal setting plans including families and carers and being responsive to any communication barriers.
  • Assist in the deployment and organisation of the outpatient group phase of cardiac rehabilitation to include group exercise sessions and educational talks.
  • Being responsible for the supervision and demonstration of exercise based cardiac rehabilitation both for individual and group based sessions.
  • Ensuring adequate levels of supervision of all cardiac rehabilitation sessions in line with service policies and procedures and according to the patients care plan.
  • Assume responsibility for monitoring the progress of all patients undergoing cardiac rehabilitation, evaluating their condition and adapting their programme as necessary within department standards and protocols, seeking senior advice/assistance as required.
  • Maintain strong communication with other professionals working within the cardiac rehabilitation service and partners both within and outside the Trust, to ensure integrated team working and improve the operational delivery of services.
  • Entering confidential patient data into paperbased patient records and using computer information systems ensuring all clinical audit data is recorded as required.
  • Liaising with relevant health and leisure professionals to ensure continuity of patient care.
  • To work flexibly within the service according to service pressures and changes in service provision.
  • Undertaking any other duties commensurate with the post as determined by the service lead.
