Job Purpose:


  • Support referrals to access and navigate the health programme interventions delivered by Active Leeds.
  • Coordinate and manage staffing resources to ensure that all health programme interventions are covered and operate consistently.
  • Ensure that the Health and Wellbeing Community Coaches and Active Leeds Activators have appropriate and accurate information on people accessing their programmes.
  • Answer and support enquiries about the Health Programme Team.
  • Ensure that all data is collected from healthbased interventions, stored correctly and is easily accessible for reports to the commissioners.
  • Support projects that increase skills, knowledge and capacity for physical activity in communities.

Responsibilities for Grade:


  • Ensure that all referrals to Health Programmes are allocated the correct intervention and that each referral has appropriate support from the Health Programme Team.
  • Liaise with Adult Social Care and Health Professionals to support people accessing Health Programmes.
  • Support people that have selfreferred to obtain the correct information needed by the Health Programme Team to ensure personal safety and that the interventions can be evaluated.
  • To support the Health Programme Manager, Leeds Let's Get Active Project Lead and Active Leeds Development Officer (Health) in coordinating delivery of Health Programmes.
  • Ensure that online, print and social media information for the Health Programme Team is correct and current.
  • Lead on the creation of referral pathways with stakeholder to ensure that referrals / recommendations can access health programmes without unnecessary barriers. Where barriers exist, the role will engage professionals and customers to provide initiative solutions to allow greater accessibility to health programmes from target audiences.
  • To maintain contact with people participating on the health interventions and support them through the Health and Wellbeing Coaches or signposting to other appropriate lifestyle services.
  • To ensure all personal information about referrals is stored safely and correctly according to GDPR guidelines. All Information shared with the Health Programme Team is transferred by secure means and that any information shared with commissioners is anonymised
  • Ensure that the health based interventions have a suitably qualified and trained member of staff instructing or coaching.
  • To support the Health Programme Team to manage any referral software and maintain the monitoring and evaluation database.
  • Support the delivery of the ongoing training programme for Council Staff, volunteers and health/exercise professionals such as Walk Leader Training, Make Every Contact Count, and Dementia Friends.
  • Support physical activity interventions delivered by the team to increase capacity of the sessions and ensure correct data collection, to achieve targets agreed by Commissioners
  • To lead and manage Health and Wellbeing Coaches and volunteers recruited by the Health Programme Team.
  • Responsible for the ordering of equipment and specific training required by the team and stakeholders.
  • To undertake any other duties which may be assigned from time to time and that are commensurate with the grade of the post.
  • To carry out the above duties in compliance with the Council's policies, e.g. Equal Opportunities, Green Strategy, etc.

Qualifications:
Educated to NVQ level 3 or equivalent

Method of Assessment will be through one or more of the following Application Form, Test, Interview or Certificate.


Skills Required:


Able to communicate with a wide range of people individually and in groups to motivate people to change their behaviour towards an active healthy lifestyle.

Ability to communicate effectively with partners such as Health and Social Care professionals in a wide range of organisations and transport providers

Able to empathise with people that have health conditions that physical activity can improve their quality of life.

Ability to work as part of a team and on own initiative.

Demonstrate good organisational and administration skills.

Able to carry out basic research and provide monitoring and evaluation reports using computer based programmes.

Able to plan work in advance to ensure a consistent service can be provided to referrals.

Able to manage a small budget for equipment and supplies for the team


Knowledge Required
Educated to NVQ level 3 or equivalent

Knowledge of the benefits of physical activity & how this can lead to a better lifestyle & improved quality of life

Knowledge of managing participants in health based intervention.
