We have a fabulous job opportunity for you to join our awesome (and growing) team Our new Advanced Social Prescriber will expand on our Community Wellbeing service and take a holistic approach to empower people in taking control of their health and wellbeing,building resilience in individuals and the community.

We connect people to community groups, as well as statutory services for practical and emotional support.

You'll support people who are high intensity users that access a range of services across the Gloucestershire Integrated Care System (ICS) including NHS111, acute hospital services, out of hours, mental health, wellbeing and ambulance services.

You'll providea coordinated and alternative care approach.
You'll provide meaningful and consistent psycho-social support and adopt a health coach and motivational approach. You'll signpost individuals and provide appropriate and personalised care plans.

You'll also support the avoidance of unnecessary acute serviceadmissions in response to a crisis of escalating needs for these individuals.

You'll work remotely and provide regular 'check ins', welfare checks and support our customers to achieve their goals and transition to other services.

You'll also focus on supporting people that are not accessing scheduled services and rely heavily on unscheduled services for their health care.

Those referred into the service will be contacted by phone and assessed using a personalised approach to uncoverany underlying reason for accessing unscheduled care.

This may reveal a range of complaints; social issues combined with alcohol dependency, mental health, criminal justice and potentially some extremely complex medical presentations.

Where it is identified that the service is not able to support the High IntensityUser due to the complexity of their presentation, the individual will be referred back to Gloucestershire Health and Care Services.
