Welcome to Aquarius as an Outreach & Transitions Practitioner.

Right now, one of our partners, Aquarius Action Projects, is looking for Practitioner to join their team.

Solihull Integrated Addiction Service (SIAS) is a partnership between four organisations jointly responsible for the delivery of the drug, alcohol, homelessness and gambling services, in the Borough of Solihull.

We offer services for adults who use or are affected by substance use, homelessness and gambling.

This role is a split role consisting of a Transitions Worker and an Outreach Worker.


The aim of the Transitions worker is to deliver high quality support and ensure key services are available to our clients aged 18-25.


The role of the Outreach Worker is to ensure that clients are supported to access key services through overcoming obstacles in their path.

The role entails reaching out to hard to engage clients who may have missed assessment slots for various reasons and offering an alternative to having to come on site.

We want to take treatment into the community so offering home visit assessment, prison in reach visits on preparation for their release or meeting in place of safety for example Library or probation office.


In this shared role, you will be working both within the community and office based setting, completing treatment assessments, home visits/outreach visits, prison visits, multi-agency working with (WMP, CMHT, GPs, probation and other supporting services) and being co-located to work along key professionals and access target groups.


Alongside getting to know people in a non-judgemental setting and treating them with empathy we hope to help break the cycle of clients entering and leaving treatment without the right support, letting them know they have choices and support to achieve their goals.

We will be introducing the SIAS service and all it has to offer and continued support from ourselves and partners that will help to build confidence, self-esteem and choice.


Your challenge? As a Practitioner you will be working specifically with clients to support the delivery of interventions and to empower change in individuals.

You will support people who are vulnerably housed or homeless, supporting in outreach and working within various locations to support clients in addressing their substance needs.

Due to the nature of the role, a driving licence and access to a car is essential.

The starting salary for this post is £21,881 with progression to £24,240 per annum.

This is a permanent full time role requiring the post holder to work 37 hours per week.
