Permanent

I'm excited to advertise for a Health and Well-coach to join a well-known PCN group in the Blackburn area.

They are looking for an experienced Health and Well-being Coach to join them on a permanent basis, ideally working up to full-time hours.

In return, you'll be entitled to

  • Salary Band 4/5 (NEG, DOE)

You'll be required to advise and implement patients personalised health and well-being plans. You'll be working with the wider clinical and non-clinical to help devise and implement health and well-being plans.
