SALARY:
£26,000 a year inclusive of London weighting


Context:


North Lewisham Primary Care Network (NLPCN) has nine GP surgeries and serves over 85,000 residents in the vibrant south-east London borough of Lewisham.

We have a diverse population and many are from Black, Asian and ethnically diverse groups, and the area has higher-than-average levels of social deprivation.

Many of those living in the area have poorer health outcomes compared to other groups, both across Lewisham and England.

These health inequalities are partly driven by differences in the uptake and provision of health and social care services, but are also substantially influenced by the structural determinants of health, including education, housing, physical work environment, and poverty.


The North Lewisham Primary Care Network (NLPCN) Lifestyle medicine service was co-designed with the local community and aims to provide people with the tools and knowledge to pro-actively improve the way they manage their health conditions.

The service supports people to build on patient activation in managing their health to improve their health outcomes and quality of life.

Lifestyle medicine is an evidence-based approach to preventing, treating and reversing chronic disease. It uses the 6 pillars of lifestyle medicine to improve patient wellbeing, quality of life and reduce disease burden.


This role would involve providing one-to-one health and wellbeing coaching and also group coaching sessions where each pillar of lifestyle medicine is discussed.


The Role:


Health and wellbeing coaching aims to provide support to patients to take pro-active steps to improve the way they manage their physical and health conditions, based on what matters to them.

You will support people to develop their knowledge, skills and confidence and to build their patient activation in managing their health.

Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self

  • Management, modelling the coaching approach in their work.

Main Duties and Responsibilities:


  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload.
  • Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access.
  • Work sensitively with people, their families and carers to gather key information, enabling tracking of the impact of health and wellbeing coaching on their health and wellbeing.
  • Encourage people, their families and carers to provide feedback and to share their stories about the impact of health coaching on their lives.
  • Attend and contribute to team, practice, and PCN meetings and events as required by the service.
  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by
individuals in the caseload.

  • Work as part of a multidisciplinary multiagency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self
  • Management, modelling the coaching approach in their work.
  • Promote and raise awareness of the lifestyle medicine service particularly to groups and communities that experience barriers to access.
  • Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.
  • Participate in regular health coaching supervision and continual learning.
  • Collect service user experience and impact of health coaching as part of the delivery of personalised care.
  • Participate and collect information that measures the impact of health coaching as an intervention that supports embedding personalised care into local health systems e.g. collect data entry relating to the health coaching activity in GP, Local Authority, and hospital clinical systems or other systems, as required.

Supervision & Training:


  • Undertake all mandatory training and induction programmes
  • Engage in regular performance reviews with your clinical supervisor and line manager address any additional training needs identified
  • Take responsibility for the identification of your own professional development needs and goals

Education:
