£27,449.52 to £31,529.41, dependent on experience plus Inner London Weighting (£4,072.06)

About the Role:


  • In your role as a Homeless Recovery Coordinator, you will:
  • Establish effective communication and referral pathways for individuals who are homeless and in contact with other health and care agencies.
  • Attend relevant multidisciplinary team meetings to support links to health and social care services, peer support networks, community assets, and mutual aid.
  • Collaborate closely with the local authority, the health and social care sector, partners, and any provisions linked to homelessness to improve joint working.
  • Liaise with other services involved in the care and support of individuals to ensure the appropriate and secure transfer of all care needs.
  • Coordinate the transfer of any accommodated outofarea individuals, including referral to and liaison with their new drug and alcohol service.
  • Enhance partnership working with primary care, mental health services, housing, and other voluntary organisations supporting rough sleepers.
  • Ensure there are culturally appropriate interventions for hardtoreach individuals from the Eastern European and Sub-Saharan African rough sleeping communities.
  • Utilise networks and pathways to broker support from strategic partnerships and other providers.
  • Ensure continuity of care for those moving out of the area.
  • Hold cases for a minimum of 46 weeks following a transition to ensure the service user moves between the two services successfully.

About You:


To excel in this role, you should:

  • Focus on engaging with people who have a history of rough sleeping and substance misuse.
  • Have some experience in engaging with individuals with multiple and/or complex needs and demonstrate a deter
