About the Role:


Broxbourne Alliance
has an opportunity for a health and wellbeing coach join our primary care network (PCN), to support our 4 GP practices across the
area, on a full-time basis for 37.5 hours per week, where you will be part of a strong personalised care team.


You will be supporting people with lower levels of patient activation, developing the knowledge, skills, and confidence, by managing their health and wellbeing, increasing patient ability to access and utilise community support whilst providing access to self-management education, peer support and social prescribing.

You will be considering the _'_whole person_'_ in addressing existing issues and encouraging proactive prevention of new illnesses.

By using this approach you will appreciate patients' circumstances in a non-judgemental manner, resulting in the need for positive communication and negotiation skills that will be supporting the patients personal choices.

The following are the core responsibilities, this will be dependent upon factors such as workload and staffing levels:

  • working as part of a multi-disciplinary team in a patient facing role to assess and treat patients using expert knowledge.
- be responsible for the assessing new patients with subsequent production and completion of individual care plans.
- coaching and motivating patients through multiple sessions, identifying their needs setting goals, and supporting them to implement their personalised health and care plan.
- providing personalised support to individuals, their families and carers, ensuring that they are active participants in their own healthcare, empowering them to take more control in managing their own health and well-being, living independently and improving their health outcomes
- being aware of limitations and, as required, being responsible for referring patients who require the intervention of other healthcare professionals.
- working with patients and understanding their level of knowledge, skills and confidence when engaging with their health and well-being.
- providing support to local community groups and work with other health, social care and voluntary sector providers, supporting patients' health and well-being holistically.
- identifying risk factors and categorising patients into appropriate risk banding.
- being responsible for the day-to-day planning of personal workloads
- liaising with the practices and, where practicable, standardising processes across the PCN.
- receiving referrals and directing patients from triage services and other clinicians.
- actively signposting patients to the correct healthcare professional.
- maintaining accurate clinical records of all patient consultations and related work.
- promoting health coaching and social prescribing amongst the PCN and networking practices, supporting colleagues to improve their skills and understanding of personalised care.
- participating in the management of patient complaints when requested to do so and participating in the identification of any necessary learning.
- contributing to public health campaigns (e.g. flu clinics) through advice or direct care.

Personalised Care Institute: 1 year or equivalent (preferred)

  • NVQ in Health and Social Care (preferred)
- be an experienced health and wellbeing professional, confident to deliver support and advice to a range of patients with diverse health needs.
- have strong experience in supporting patients with long-term conditions and a lack of knowledge, or skills, to improve and manage their own health and wellbeing effectively.
- have excellent interpersonal and communication skills.
- be confident to be involved in multidisciplinary meetings.
