An exciting opportunity has arisen to join a happy caring and dynamic inclusive team on Chilgrove Discharge Unit. We are looking for a calm, caring enthusiastic person to undertake the new role of activities coordinator.

Liaising with our multi disciplinary team, you will be organising and engaging our elderly, frail patients in activities either on a one to one basis or small groups.

Providing an activities programme to meet the needs of our patients. You can be autonomous and create within this role improving our patient experience whilst there are in hospital. You will need to have experience in care and have a good understanding of frailty and dementia.

Providing an activity programme for patients ward based.

Engaging in activities with patients on a one to one basis or group based.

  • Responsible for own actions, ensuring all patient interactions/observations are communicated to trained staff. Documenting summary in each patients notes of activity and engagement.
  • Show dignity and respect in dealings with patients, staff and visitors.
  • To assist with mealtimes and feeding patients where required.
  • Organise own daily workload e.g. identification of suitable patients to take part in activities in conjunction with discussion with nurse in charge.
  • Provide instruction (verbal and non-verbal) to patients on a range of activities as part of the activities programme.
  • To develop a rapport with patients, to encourage and motivate them to participate in activity (either group or individual work).
  • To contribute to the development and implementation of a creative and innovative activities programme responsive to the varying needs of the patient group.
  • To observe general behaviour, abilities and responses of patients to the multi-disciplinary team and document within patient record.
  • Support staff and patients exposed to difficult situations arising in the clinical area e.g. after receipt of bad news or following an unexpected event.
  • Maintain a clean, safe environment ensuring adherence to Trust standards of cleanliness, hygiene, and infection control at all times.
  • Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
