At Cherish UK are currently recruiting a Care Assessor to work as part of our Rochdale Care Team.

As Care Assessor your main responsibilities are:

  • To ensure the continuity of care to our clients by assessing their dynamic needs
  • To ensure that our personcentred Care Plans are always uptodate
  • Delivering a high quality care service
  • Liaising with care professionals, including the Local Authority and Social Workers
  • Offering support to our Support Workers with regard to their duties outlined in the client's care plan
  • Liaise with our Care Coordinators to ensue staff/client compatibility
  • On call duties will also be a responsibility of the role.
You must be dedicated and hard-working and it is essential that you have a driver's licence. Experience working with People Planner computer system would be an advantage. You have the opportunity to undertake domiciliary care duties outside of the working pattern for this role.

  • Previous experience of working as a Care Assessor
  • Willing to work independently and as part of a team
  • Excellent attention to detail
  • Superb administrative skills
  • Experience working with the Microsoft Word Package
  • Has a great clientfocused manner
  • Is able to drive, due to the travelling involved
  • NVQ Level 3 in Health and Social Care is preferred
