Brent - NW London

£20/hr


Akton recruitment are currently working with a Local Authority in Brent who are looking to recruit a care assesor to join their team.

We are looking for someone who has relevant experience of working directly with people in an empowering way.

You will be approachable and have a warm, friendly, outgoing nature and be committed to working in a way that celebrates a person's strengths despite what they might be going through in their life.

You will be able to work on your own but with the support of the health team and wider community.


You will 'walk alongside' people, building on their strengths, to achieve their vision of a better life, assisting that person to deal with the things in their life they want to change.

These can be related to housing, mental health, physical ill health, domestic violence, debt and money problems, isolation, sudden change in family circumstances and a wide range of challenges.

Some people just want some information and signposting.

You will be working closely with health team, as well as a range of in-house services, the voluntary and community sector and charitable organisations to find practical, non-service solutions to problems wherever possible.

You will be required to support in building more welcoming, inclusive and supportive communities. The role requires community working and some administrative/recording time.


Job Purpose:


  • To carry out social care and joint health assessments.
  • Devise appropriate comprehensive support plans in order to meet identified needs
  • Ability to evaluate and highlight varying needs for the purpose of different types of care support options, traditional care, reablement or supported living
  • Participate in care discussions and multidisciplinary assessments regarding customers
  • To be creative in determining appropriate services to meet varying needs
  • Ability to research both formal and informal services to meet needs
  • To monitor and review packages of care
  • To work in partnerships with other statutory, voluntary and private sector organisations. Contribute to the development of the Team
  • To undertake actions in relation to Safeguarding Adults, as required within the locally agreed Adult Safeguarding policies

Experience:


  • Experience of working in Social Care Services or similar organisations such as the NHS or Voluntary Sector.
  • Experience of multi disciplinary agency working within a multicultural environment.
  • Experience of having assessed the needs of people, devise reablement/outcomes/goal setting for the purpose of support planning.
  • Experience of working with vulnerable adults with learning disabilities in a social care context, using acquired skills and judgement in order to build professional relationships, communicate effectively, have appropriate professional attitude when representing the council.

Skills and Abilities:


  • Good customer care skills demonstrating ability to within the Council's Customer Care Procedures.
  • Good interpersonal skills, demonstrating the ability to work with colleagues and other external agencies.
  • Ability to plan own work, work under pressure and use of effective methods deadlines.
  • Good communication skills (verbal & written).
  • Ability to collate information and produce relevant assessments; support and review action plans and other relevant reports.
