Description
Applicants should be enthusiastic with an awareness of and committed to the modernisation of the service. They should be able to work well within a team and also alone.

You must be able to travel between patients homes both within towns and rural areas in a timely manner and welsh speaking is desirable


Direct Patient Care as allocated by a trained nurse after appropriate training and within the framework of a plan of care implemented and reassessed by a registered nurse.

Organising tasks in a flexible manner according to priority of needs.

Assistance and support to the patient in maintaining an acceptable quality of life.

Maintain accurate and legible documentation in accordance with Health Board Policy, ensuring confidentiality at all times.

Order HSDU, stores, check delivery, ensuring efficient use of resources.

Assist with any clerical duties as appropriate.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
