Hywel Dda University Health Board are looking to recruit a full time Community Mental Health Practitioner with a qualification and registration as a RMN nurse, Social Worker or Occupational Therapist.
The successful applicant will join the Community Mental Health Team, based at Haven Way, South Pembrokeshire Hospital, Pembroke Dock and Bloomfields in Narberth.
The hours will be worked in 7.5hr shifts over a 9am to 5pm shift pattern rota also covering weekends possible in the future.

Our service operates 7 days a week and works through an Assessment & Recovery approach with people accessing Secondary Mental Health Services.
The post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care for a defined clinical caseload including the role of a Care Co-ordinator within Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder will require effective communication skills and be able to develop positive working relationships between all healthcare professionals and statutory and voluntary agencies, maintaining good communication across all boundaries.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

- Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
- 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
- Numerous locations providing mental health and learning disabilities services;
- Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
