Main function of the job:


To support service users with, mental illness, learning disabilities, dementia and additional complex needs, to live the life they want within the service.

You will support service-users in all aspects of daily living and community participation and be an instrumental part of their daily lives by giving regular one to one support which will include individual support by suggesting and leading activities that will aid service-users in leading a valued and fulfilled life.

Encouraging and empowering service users to reach their goals and aspirations. Maximising service user's potential to live a healthy and active lifestyles as independently as possible.

You will assist by providing support and guidance in various ways, including:

Shopping with service users who need care and support to attend appointments if needed, developing everyday skills such as how to make a cup of tea or prepare and cook a meal safely, or simply being in their home environment and helping to keep their personal space clean and tidy.

You will be required from time to time to support service users to use the telephone to call various agencies with regards to benefits, Doctors appointments, CPN and Social Workers.

Daily duties will include

  • Recording daily notes and appointments
  • Participating in handover including countdown of medications and checking MAR charts
  • Dispensing medication and keeping clear records of medication given
  • Recording incidents and reporting to registered manager/on call manager
  • Daily duties will also include cleaning communal areas within the home and adhering to health and safety measures.
  • Support to help keep service users personal space clean and tidy


You will be required to maintain care skills, and undertake such training and development as may from time-to-time be required to maintain that currency of practice.


You will work in teams with other professionals, including managers, social workers, other outreach and community support workers, drug action groups, youth offending services, the police, education authority and other health authorities.


You will be required to provide care in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Manager, and within the financial plans agreed from time-to-time.

(Note:
In addition to these functions employees are required to carry out such duties as may reasonably be required).


Location:


Redburn House Care Home, but you may be relocated within the local vicinity and other services ran by company as and when is required


Main Duties (not in any order of priority):


Working within the organisation:


1. Develop effective working relationships with the other employees within The Home.


2. Support an open, positive and inclusive working culture.


3. Participate in the development of The Home's policies.


4. Participate in evaluation of The Home against agreed organisational goals, business, and quality objectives.


5. Work to establish effective employer-employee relationships.


6. Minimise legal risks.


7. Participate in the maintenance of The Home's management information systems.


8. Assist in the formulation and implementation of care policies and procedures.


9. Assist in the implementation and maintenance of the standards required by legislation related to the registration of The Home.


10. Act within The Home's budget based on The Home's objectives and within the projected revenue.


11. Work in a cost-effective manner.


12. Be involved in the implementation and maintenance of The Home's quality assurance programme.


13. Assist in the design and administration of an evaluation of the care standards and care service provision.


14. Systematically solve day-to-day problematical issues which arise.

Care Services:


1. Assist in the development of the philosophy, goals and objectives for the care practice.


2. Assist in the assessment of the effectiveness of care implementation and delivery


3. Implement action to meet and maintain care standards.


4. Work in cooperation with members of multi-disciplinary health teams in order to maximise opportunities for Service User therapeutic care.


5. Ensure Service User rights are protected.


6. Encourage a model of self-care and Service User rehabilitation.


7. Record relevant activities in Care Plans.


8. Evaluate standards of care competence.


Professional Long Term Care Leadership:


1. Encourage innovative methods for the delivery of care.


2. Encourage health promotion within care strategies.
