Highly skilled Nursing team,Pharmacist non-medical prescriber and social prescribing.

  • Responsible for the administration, coordination, admin IT systems and monitoring of projects
  • Assist as clinical coder to achieve QOF; LCS; DES; patient targets through:
  • Work with our GP's and other primary care professionals in order to identify and manage a caseload of patients with specific needs, flagging any concerns to the relevant GP and referring to other health professionals as appropriate.
  • Utilise population health intelligence and reporting to identify and work with highrisk patient populations and specific cohorts; such as those with long term conditions, learning disabilities, those requiring palliative care or safeguarding concerns.
  • Work with clinicians to facilitate referrals
  • Assist with coordinating clinics and recalls for flu, routine immunisations, SMI health checks, diabetes checks and reviews.
  • Liaise with care homes, community matrons and district nurses as and when needed.
  • Code and log relevant information in EMIS.
  • Ensuring that our service complies with CQC requirements
  • Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles
  • Assist Practice Manager/Admin Team Leader with new staff inductions and external contractors.
  • Assist Practice Manager/Admin Coordinator with any other reasonable duties as required of the post.
  • To provide support and training for current and new staff in resolving simple problems with PCs and printers
  • To liaise with IT support department to resolve other hardware and software issues.
  • Ensure the effective use of ERS, EPS, Docman, Dart and other IT programmes
