As a Community Support Workers you would join our multidisciplinary Homefirst team based in Kerrier but your base can be flexible according to your preferred location.

The post holder will deliver reablement as part of a collaboration between community, acute and social care providers.


You will be part of a dedicated team and will be provided with bespoke training to support your development in this role.

You will need to demonstrate good communication skills and a positive approach to older people who may have communication difficulties or who may have challenging and complex health needs including frailty.


The team provide a 8am -8pm, 7 days a week service to community patients and we are happy to discuss flexibility around hours and days worked.


The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting.


The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital.


The post holder will be expected to deliver packages of care to patients in their own home; this may also include Residential homes and nursing homes and we offer a mileage allowance for journeys in your own car and access to pool cars where availability allows.


The post holder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service however, we are happy to discuss flexible opportunities with you.
