Full time 37.5 hours per week.

The future of Health Care is changing and the Admission Avoidance Team is crucial to delivering the vision, therefore we are expanding our service to enable us to deliver innovations such as the Virtual Ward and Hospital at Home model of care.

The team needs to recruit 2 x band 7 clinical practitioners on a fixed term contract or secondment until 31/03/24.

These new posts will need to engage closely with acute staff as well as community colleagues and social care staff to coordinate and provide high quality care in the community.
