Job Advert Trainee ACP in Stroke

We are seeking an enthusiastic team member to join the team as a Trainee ACP in Stroke.


It is anticipated that the Advanced Clinical Practitioner (ACP) role will augment the existing Stroke team and enhance the quality, efficiency and flow of our services for patients.


This is an exciting opportunity to gain further professional development by undertaking a formal qualification to progress to an ACP (following successful completion of the 3 year programme), gain the relevant skills and competence to autonomously work in providing inpatient and outpatient care, as well as leading on transformation work on the Stroke pathway.


The post holder would be responsible for enabling Stroke specialist outreach, assessment, education and advice across the trust to improve early identification of Stroke and liaison between relevant teams to improve performance against these KPIs and patient outcomes.


The post holder would also work on further improving the hospital/community interface with earlier discharge of patients from the inpatient bed base.

This ACP role would be uniquely placed to link collaboratively and to enable integration of acute and community teams to complete this quality improvement work.
