Description
The post holder will undertake a key worker role, alongside an assessment and formulation role within the team.

The Living Well Service will enable timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services.

The team will be multidisciplinary and will also include third sector organizations. The areas that this team will cover (second Living Well Team) are Turton, Halliwell, Horwich, Westhoughton & Chorley Roads.
