An exciting opportunity has arisen at the Community Mental Health Teams in Salford.

We are looking to recruit a Registered Nurse Associate/ Assistant Practitioner or Trainee Nursing Associate to work within the CMHTs, focusing on undertaking Physical Health Care assessments and monitoring physical health, alongside other service user and carer interventions.


Salford CMHTs are fast paced, dynamic, supportive, well performing teams within the Salford directorate of GMMH covering a wide geographical area and varying service user and carer needs.

The successful applicant will have some experience of working with service users with mental health issues.


The post holder alongside the physical health nurse will be able to hold physical health clinics and work as part of a busy multi-disciplinary team supporting service users address their physical and mental health needs.


  • Health Promotion
  • Physical Health Improvement Tool
  • Support with Clozapine, Mood stabiliser and Depot clinics if required
  • Undertake Venepuncture as required within the PHIT Tool or directed by the senior nurse or medic.
  • Undertake ECG as required within the PHIT Tool or directed by the senior nurse or medic.
  • Supporting annual flu vaccination clinics if required

The Registered Nurse Associate/ Qualified Assistant Practitioner for Physical Health will deliver high level, quality care and evidence-based interventions within a multi-disciplinary team.

The Assistant Practitioner will provide planned care and be able to take appropriate action to achieve the planned aims and objectives.

The Registered Nurse Associate/ Qualified Assistant Practitioner for Physical Health will be competent in:

  • Health Promotion
Discussion and signposting with service users around lifestyle, smoking, diet, exercise, diabetes, cholesterol, alcohol etc. Offer brief interventions and make referrals to relevant services such as weight management or exercise.

  • Physical Health Improvement Tool
Ensure PHIT targets for metabolic screening completed yearly for service users who meet the PHIT criteria. Ensure PHIT assessments completed and results inputted and shared with service user, GP, care coordinator/medic. Ensure all interventions recorded through the progress note tile on PARIS. Refer to specialist nurses e.g. drugs and alcohol as appropriate and liaise with community staff.

Ensure all service users who meet the PHIT criteria have PHIT reviewed 12 monthly Reviewed at 6 months and then every 6 months thereafter.

Assisting in the achievement of CQUIN and KPI targets for physical health care.

  • Undertake Venepuncture as required within the PHIT Tool or directed by the senior nurse or medic.
  • Undertake ECG as required within the PHIT Tool or directed by the senior nurse or medic.
  • Medical devices
Undertake quality checks where required. Ensure medical device faults are reported and followed up. Report all problems to Senior Nurse Practitioner

  • Supporting annual flu vaccination clinics
Support qualified staff with flu vaccinations clinic. Give influenza vaccine under supervision of Senior Nurse Practitioner.
