Description

Key responsibilities 1. Manage patient sign posting and referral for services within the various practices 2. Ensure regular and consistent communication with the referrer regarding patient progress and any complications or guidance 3. Proactively participate in health promotion and screening programmes for patients 4.

Identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids 5.

Undertake work in line with PCN directed priorities. 6. There are numerous work streams under our contract and the care coordinator will work closely with the Clinical Director, the Strategic and Operations Director, as well as Practice Managers, GPs and other staff within the various PCN practices
