Framework has an exciting opportunity for a role in Social Care working 3 days a week 9am-5pm. The Support Planner will join a small and friendly team based in Nottingham County.


Elizabeth House Move On is the move-on accommodation from Elizabeth House and provides accommodation and support for homeless people from Broxtowe, Gedling, and Rushcliffe.

The office is based at Elizabeth House on Mansfield Rd, however our properties are based in the same 3 boroughs and as such the successful applicant will preferably need a full driving licence and access to a car for work purposes.

Please note support planners are required to use the core service and or work from home while delivering a high standard of support and guidance to our client group.


Duties & Responsibilities

  • To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs
  • You will have caseloads with tenant's in properties across the 3 boroughs (mileage system for car drivers and public transport paid).
  • Prepare Support plans.
  • Signpost Tenant's onto appropriate services for substance misuse management & health.
  • Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
  • To liaise appropriately with key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
