We are pleased to announce an exciting opportunity which has arisen for a Health and Wellbeing Coach to join our developing multidisciplinary team at NMWP Primary Care Network.


As an experienced Health & Wellbeing Coach, you will be required to support our practices by utilising your health coaching skills to support individuals with lower levels of patient activation.

You will help them to develop their knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.

You may also provide access to self-management education, peer support, and social prescribing.


You will work as part of the Health & Wellbeing team and will support several practices, tailoring your work to the needs of their populations and to your own interests/skills.

As the team expands, we hope to work with the wider multidisciplinary team to deliver more health promotion events and create opportunities for peer support for patients.


KEY RESPONSIBILITIES


Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.

Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently and improve their health outcomes through:

  • Providing interventions
  • Supporting people to establish and attain goals set by the person based on what is important to them
  • Working with the longterm conditions (LTC) teams to deliver specific lifestyle advice (prediabetes, hypercholesterolaemia, hypertension)
  • Working with the social prescribing service to connect patients to communitybased activities
  • Working to decrease the prevalence of treatable LTCs by undertaking health checks if required
  • Delivering weight management group programmes
  • Inputting data including appropriate disease and referral coding
  • Providing support to local community groups and working with other health, social care and voluntary sector providers to support the patients; health and wellbeing holistically
  • Helping to establish and coordinate patient groups to aid selfmanagement, peer support, and the creation of patient communities
  • Undertaking all the necessary training and induction for the role
  • Raising awareness within the PCN of shared decision making and decision support tools, and supporting people in shared decisionmaking conversations
  • Utilising existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching
  • Working with the PCN and practices towards a number of different outcome and quality improvement measures including QOF, QIPP and IIF

Experience:


  • NVQ Level 3 Health Trainer qualification or other relevant professional academic qualification
  • Educated to degree level or related discipline
  • Training in motivational interviewing/coaching
  • Training in delivering psychological interventions
  • Experience of project management in a healthcare or voluntary setting

Skills and Knowledge:


  • Understanding of the importance and process of helping people with longterm conditions to develop their knowledge, skills and confidence in managing their health and the range of models and tools available
  • Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving
  • Demonstrable skills in supporting behaviour change
  • Excellent group and onetoone facilitation skills including conflict resolution
  • Skilled in active and reflective listening, building trust and rapport quickly
  • Excellent communication and presentation skills
