A rare and exciting opportunity has arisen, giving you the opportunity to join our expanding Adult Social Adult Social Care Department.

Our focus is on supporting vulnerable adults over 18 with a wide range of needs, including physical disabilities, learning difficulties, autism, substance use, domestic abuse, mental health and homelessness.

If you have experience in these areas or a keen interest in supporting and learning more about these needs, this could be the role you are looking for.

Your role will be undertaking care needs assessments, reviewing services and developing support plans to meet assessed needs. You will also be working to support and engage adults, using trauma informed principals.


This will involve offering practical advice and support to access services in addition to gathering relevant assessment information and developing a relationship of trust with the individual.


Due to the nature of this role, you will hold an interesting and varied caseload so being adaptable and flexible is key to your success.
