This is an Office Based role


We now have an exciting opportunity for a Care Liaison Worker to join a team of committed professionals in our Community Support Services Team in an exciting role that will provide information, advice and practical and emotional support to older adults, who are in the next stage of the care journey.


The role will directly help and support clients and families to make an informed choice about care, funding and how to choose the right support for them.

You will need a good understanding of multi-disciplinary working procedures and a firm understanding of Social Care Regulations such as the Mental Capacity Act


Be able to raise awareness of the availability of this service, and to identify any barriers to individuals accessing services and activities and work with individuals to overcome these.

You will need to be have good record keeping skills to ensure that electronic client records, activity, progress towards outcomes, and other monitoring and information is completed accurately and in a timely manner

Safeguarding, monitoring and reporting are essential to this role.

From £27,500.00 per year
