Lewisham Community Mental Health Team (CMHT N3) have undergone service transformation.The transformation is about enhancing the quality of our service delivery, improving the experiences of those using our services, maintaining staff morale and to ensure services are meeting the NHS Long Term Plan.
It is an exciting time to be part of this transformation in Lewisham, getting involved in improved ways of working due to investment in resources.
The Community Mental Health Team (CMHT) is a generic team integrated with Lewisham Social Care in working with all adults experiencing severe and enduring mental illness.
The new CMHTs are are no longer based on diagnosis and work with all individuals from a locality who have mental health needs that require support from secondary care.
We are looking for an enthusiastic mental health professional / care coordinator with a professional background in either Nursing, Social work or Occupational Therapy to join this highly experienced and supportive Multi-disciplinary CMHT, covering the South of the borough.
The post holder will work as Care Coordinator / Community Mental Health Nurse for a caseload of service users in the local community who have complex mental health difficulties, and work with their carers where applicable.
The post holder must be able to formulate flexible, robust and holistic care plans with service users and/or carers, to work well under pressure, be experienced in dynamic risk assessment, and to have excellent communication skills.

Job Purpose:
The post holder will deliver, or advise on the delivery of, high quality safe care, ensuring consistently high standards of practice and clinical excellence which comply with their registered professional body and code of professional conduct.
As a Community Mental Health Professional you will promote and develop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care.
You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment.
As a senior member of staff working within the South London Mental Health and Community Partnership you are required to encourage and foster collaborative and cross organisational working.

Core Responsibilities:
As a practitioner working in a community setting you will promote and deliver evidence-based care for service users with mental health conditions in their own homes and within a variety of accommodation settings.
Working autonomously within the parameters of the role leading and supporting junior staff and wider care team to implement the delivery of quality care, contributing to the physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and taking steps to address any issues that arise.
