Would you like a role working in a community setting offering variety, no two days the same, working with a range of professionals in dynamic, innovative, and supportive teams?

We have an exciting opportunity to work in our Neighbourhood teams in Leeds, alongside physiotherapists and occupational therapists as a Band 4 Therapy Assistant Practitioner.

We are looking for someone with previous experience of health and social care, with excellent communication and organisational skills, looking to further enhance their skills in person centred therapeutic rehabilitation of adults.

You will have the opportunity to organise and manage your own caseload, so we are looking for someone who can work independently within scope, using their own initiative.

We facilitate our service users to optimally manage their health conditions, promote independence with daily activities, and enable early discharge from hospital or community-based beds, working towards goals to improve health and wellbeing outcomes.
