Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role
Floating Support services work with people to sustain their housing.

We use a strength-based approach to work with people to identify what support they require and how we can address them together or / and in collaboration with others e.g.

landlords, Department of Work and pension or mental health services.

We can support people for the duration of their tenancy, or we can provide short term intervention.

We can provide resettlement support when a tenancy commences, and we can equip people with the skills to manage their home and goals independently or when they are move on ready and require mínimal support.

We are looking for a
Senior Practitioner to join the
Tenancy Sustainment Team

You will:

  • Line manage Floating Support Workers within a locality or specialist team
  • Manage and coach staff to deliver safe and personcentred support to people with support needs across dispersed community locations
  • Facilitate and lead on casework management, client allocation and client welfare interventions, ensuring our clients experience a consistent and effective service at all stages of their support
  • Promote the service to, and work in partnership with, key local services including health, housing, social care and Employment & Skills teams
  • Work collaboratively with the people we support, and with other key stakeholders, to deliver coordinated support and to achieve key outcomes
  • Contribute towards service evaluation and improvement through engagement with the people we support, with colleagues, and with other key stakeholders
To succeed as the
Senior Practitioner, you will have

  • A proven track record in leading and motivating a team to achieve key outcomes and results
  • Experience in organising the operations of a complex, integrated and busy service
  • Experience of working to a Solution focussed approach and in working with others to overcome challenges and barriers
  • A visible and approachable leadership style, building a culture of high performance and best practice within the team
  • Excellent communication skills, able to form and maintain effective working relationships with all partners
  • A proven track record of building and developing a culture of support delivery that is Trauma-Informed, Strengths-Based and Co-Produced with people we support
