Job description:
We are a growing business on the lookout for a compassionate and committed Care Coordinator to join our team in St Helen's.
- Full Time, Permanent
- Monday
- Friday, 9am 5pm
- The above includes on call payment 1 in 4 rota basis
- Bonus scheme of up to £2,000 per annum (paid quarterly)
Our Care Coordinators play an integral role in ensuring that our service users receive a high quality, complex care service that we'd be happy for our own family members to receive.
You'll be overseeing and managing a caseload of complex care packages, acting as the first point of contact for our service users, their families and other healthcare professionals.
In addition to the above, you will also:
- Respond and manage day to day changes within your care packages
- Coordinate Rota's
- Manage health care staff including reviews, appraisals and spot checks
- Schedule monthly meetings with service users
- Conduct care plan reviews
It's a fast paced and varied role and we're looking for someone who is enthusiastic and self-motivated with excellent communication and organisational skills.
Essential Skills:
- Evidence of working in a fastpaced role
- Strong organisational, IT and admin skills
- Likeminded individuals, aligned with our values
- Drivers with access to a vehicle
Desirable Skills:
- Understanding or knowledge of Complex Care
- Coordination experience (ideally within healthcare)
