The Peripatetic Patient Journey Facilitator role will be based at The Royal Shrewsbury Hospital and will be required to cover as the service demands any of the Medical wards.
Your role is to facilitate the SAFER Red2Green approach and also to assist the Nurse in Charge with daily discharges.
SAFER Red2Green aims to value patient time and improve flow by reducing delays in a patient's hospital journey.
We are therefore seeking a keen individual with a 'can-do' attitude who is able to work flexibly (not ward based) alongside the Patient Flow team to progress the patient's journey.
