Description

Are you an experienced clinical practitioner seeking a new challenge where you can make a positive difference? We are looking to appoint a Band 6 clinical practitioner to join our Learning Disabilities Assessment and Treatment Unit at Townend Court.


You will work in close partnership with adults with learning disabilities, their families, the multidisciplinary team, and relevant others to coordinate the transition and discharge plans for adults with a learning disability that are open to our service.

You will help to inform the person's needs, liaising closely with the inpatient team, community team, family and others involved in the patients care.

To oversee and coordinate the discharge plans for patients with a Learning Disability admitted to Inpatient Services.


To work alongside the patient, their family, their social worker and the rest of the MDT to assess the patient's post-discharge needs.

To collate the recommendations of own and other professional's assessments to provide clear recommendations for the social workers and commissioners regarding post-discharge needs

To be a central point of contact for families, social workers, commissioners, and other services regarding discharge plans

Support family contact whilst a patient is staying on the inpatient service.

To attend MDT meetings and core group planning meetings, ensuring good communication between all parties.


To coordinate transition plans, and to continue to be a point of contact for the first few weeks after discharge from the hospital.
