Shrewsbury

Monday to Friday 9am-5pm

Temporary - ongoing

Up to £17.06 per hour


My client in Shrewsbury require a Personal Adviser to join the leaving care team who can offer support and advice mainly to their unaccompanied asylum seeking young people but will also be supporting other care leavers.

You will manage a caseload of around 25 young people who are aged 17+ and be expected to carry out statutory duties as outlined in the Leaving Care Act 2000.


Duties include:


  • To hold a caseload of young adults who are Asylum seeking Care Leavers
  • To complete Needs assessments
  • To complete with the care leaver their Pathway Plan, to set goals and outcomes, and assist in the identification, delivery, and achievement of the goals and ensure triple planning is evident within the plan.
  • To regularly review and respond to significant changes in the needs of the Care Leaver ensuring their plans are updated to reflect the need identified
  • To advocate for Care Leavers with other statutory agencies, including Housing, Benefits agencies, Probation, Colleges and Universities, Health and Emotional Well Being Services, Adult Services and legal service etc.
  • To attend regular appointments when requested by the Care Leaver to support and obtain the best outcome desired.
  • To attend Court appointments or hearings were requested by the Care Leave
  • To assist with the setting up of Care Leavers in independent accommodation, with financial and practical arrangements.
  • To visit and spend time discussing the transition to live alone in their own accommodation.
  • To visit the Care Leaver where they are living, this will require distance travel across the county and out of the county distance travel.
  • To assist, advice and befriend the Care Leaver to ensure their transition to adulthood develops independence, knowledge and skills.
  • To organise and plan for other support services and people to be available to Care Leavers, building up their support network and introduce to relevant services and people.
