Description
This a 12 month fixed term position.


The post holder will also work in a team of pharmacists and pharmacy technicians providing expert medicines advice to the primary care Community Health and Wellbeing multi-disciplinary teams.


Collaborating with health & social care colleagues across primary and secondary care, the post holder will deliver patient focused care through innovation and commitment.

You will possess excellent communication and organisational skills, be self-motivated and able to innovate through new ways of working.

The ability to work effectively as part of a team and provide ongoing leadership to colleagues are also essential for this role.


The main duties of the role will include:

Provide guidance and support on complex medicines management issues to health & social care staff involved in the care of patients.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
