The post holder will work closely with the Maternity Risk and Governance Lead to develop and facilitate implementation of the Neonatal & Maternity Risk Management Strategy and provide specialist risk management advice to staff within the maternity directorate.


They will be responsible for; the on-going monitoring of the neonatal & maternity risk register, monitoring and investigating local and serious incidents; using root cause analysis where appropriate.

They will be proactive in their approach to risk prevention and will help to encourage an open and transparent reporting culture that enables the organisation to learn lessons and prevent incidents from reoccurring.

To deputise for the Maternity Risk/Governance Lead in their absence or as appropriate


The post holder will work within the maternity governance team and will have links with clinical and non-clinical teams across the directorate.


  • Ensure effective implementation of the directorate's Risk Management Strategy.
  • Ensure that all risk management related procedures are robust and current and reflect national risk, governance and patient safety initiatives, legislation and healthcare priorities.
  • Adapt professional knowledge to the relevant specialist field so that clinical staff, managers and heads of service are educated and supported in the implementation of any changes and new requirements with which the directorate must comply.
  • Participate and support GAP analysis process as they relate to risk management

  • Develop and implement robust local risk management practices ensuring compliance with the requirements of the Trust's and Directorate's Risk Management Strategy.
  • Promote and develop a fair and open approach where staff report incidents in a timely manner.
  • To ensure that staff within the directorate are aware of their responsibility and have appropriate access and training to support the timely reporting of incidents onto the trust system Datix.
  • Manage the Incident Reporting Policy and associated procedures throughout the Directorate, as the basis for reporting and mitigating incidents.
  • Provide support and advice to the local incident managers and the directorate incident managers in carrying out investigations of incidents and near misses including the identification and implementation of action plans and controls measures to prevent incident recurrence in line with key policies ensuring mandatory timeframes are adhered to.
  • Lead on investigations of local and serious incidents, using Root Cause Analysis (RCA) where appropriate.
  • Monitor Action plans that relate to RCAs and escalate where there may be difficulties in completing these.
  • Provide timely responses to queries from external bodies where applicable
  • Provide expert advice and leadership on matters relating to risk assessment, risk management and the promotion of assurance within the Trust.
  • Manage the Maternity Directorate Risk Register, ensuring all risks and reviewed in line with the trust policy.
  • Chair the Directorate's monthly Risk Management meeting. Assist in undertaking responsibilities in preparation for a CQC inspection and other governance related reviews.
  • Ensure learning from incident reporting and risk assessments is incorporated into an iterative process to ensure learning is assimilated across the directorate.
  • Provide expert advice, information and support to relevant directorate wide groups
  • Ensure that all moderate, severe or death harm incidents, where appropriate, are managed in line with duty of candour.
