You'll require strong analytical skills and the ability to identify those in the greatest need, working in partnership with both internal and external teams and services to seek creative solutions.

The work is largely office-based health and housing assessments with some home visiting.

It includes follow up and liaison with other health professionals, services and agencies in both determining housing need, priority, and other solutions to complex cases.

What do I need to have?

You should have a recognised health qualification (e.g. Registered nurse, Occupational Therapist) or equivalent relevant experience and experience in a range of settings. You'll be accessing information electronically and therefore it's essential that you're IT literate.
