Care Coordinator

- To ensure that the Home Care Business fulfils the requirements of the Health and Social Care Act 2008 Regulated Activities) Regulations 2014, and all other relevant legislation.

  • To participate as directed in preparing the Branch for a CQC, CCG and or Local Authority Inspections
  • As directed to ensure, that suitably experienced, appropriate and assessed personnel are supplied at all times to all Service Users
  • To allocate Service User visits/calls to Care Workers ensuring continuity at all times.
  • To ensure that all immediate calls are covered
  • To ensure that service provision is personalised to meet the needs of all Service Users.
  • All Care Workers and Service User record to be kept up to date on the designated software programme, ensuring that the care is provided to Service Users as the service requires.
  • To ensure that no calls are missed when being responsible for the out of hours service
  • As direct to carrying out assessments or spot checks
  • As directed to complete assessments and care planning for new Service Users, ensuring care plans are detailed
  • To maintain confidentiality of Service Users, families and Rhythmic Care.
  • To liaise with other members of the multidisciplinary team for example District Nurses and ensure detailed records are maintained.
  • To create and maintain good communication with all Service Users including those who may have some degree of communication difficulties
  • To maintain all written records and other information as required in accordance with Rhythmic Cares operating policies, Local Authority, CCG and legislative requirements.
  • As required to assist in the administration and delivery of home care and support services i.e. when there is no other staff available.
  • As directed to assist with the recruitment, training and assessment of other staff
  • As directed to assist with the explanation/implementation of any new procedures and/or company policies to other staff/Care Workers
  • To participate as required at Service User review meetings.
  • To demonstrate an awareness and commitment to Rhythmic Care equal opportunities policy.
  • To adhere to the policies and procedures of Rhythmic Care.
  • To attend supervision, training and assessment programmes.
  • To abide by the Health and Safety at Work Act, Food Safety Act, Electricity at Work Act, COSHH, Care Act and any other relevant legislation
  • To immediately communicate any identified risks or potential risks to the Registered Manager and or your Office Manager. Furthermore, to report and document any incident, complaint accident or untoward occurrence.
  • To use equipment as instructed and report immediately any breakage's or equipment failures etc. To ensure that damaged equipment is removed and marked as appropriate.
  • To be aware of the importance of not placing others or yourself at risk.
  • To assist as required in the collation of time sheets, rotas and work sheets in order to facilitate the operation of payroll and invoices.
  • As directed to assist in the marketing of services to both the internal and external environment
  • To act in such a manner as to promote and enhance the welfare of Service Users, employees and Rhythmic Care ensuring that no act or omission results in Rhythmic Care being brought into disrepute.
  • To communicate to the Registered Manager any information pertaining to the Service Users.
  • To maintain at all times a professional relationship with Service Users, their representatives/next of kin and other employees.
  • To participate in a work rota including weekends as required
  • To coordinate the out of office hours service
  • To communicate to the Registered Manager/ Office Manager information that may be of benefit and/or detrimental to the business as a whole.

Salary:
£23,000.00-£28,000.00 per year
