We are seeking applicants with outstanding skills to join a new service working within Newcastle/Gateshead PCNs.

The successful applicant will be a primary care mental health practitioner with a focus on providing triage, assessment, and brief intervention to adults.

Applicants must have excellent communication and organisational skills, and will be required to work collaboratively with partners within GP practices, demonstrating the effectiveness of this service through the consistent use of appropriate outcome measures.


This is a very exciting, diverse, and autonomous role, as such it will be vital to develop and maintain positive working relationships with the primary and secondary care teams, as well as VCSE's.

With this in mind the post is based within a Primary Care Network, and as such you will be based within GP practices within that network on a full time basis.

This is to ensure visibility and development of professional relationships, as well as to be on hand to offer and seek advice.

You will also be supported by the PCMHW team, community clinical manager, as well as the Primary Care Leads.


As part of the PCMH role, you will be expected to undertake an holistic assessment for adults who present with mental health difficulties such as anxiety, depression, eating disorders, psychosis, and neurodevelopmental difficulties.

The assessment process may work as a first contact role, or you may receive referrals from primary care staff following their assessment.

Following assessment, the post holder will provide where indicated intervention, signposting to alterative services, and/or make an onward referral to appropriate services.


This is a fast paced autonomous role, with a need for the holder to be able to make appropriate evidence based decisions, keep robust, timely and concise records and provide appropriate statistical information and reports as required.
