An exciting opportunity has arisen for an experienced nurse to join our team in the role of Head of Patient Co-ordination Services at Royal Trinity Hospice.

The Head of Patient Co-ordination Services will be responsible for the strategic and operational leadership and management of the Admission and Referrals services and Wandsworth End of Life Care Coordination services (WEoLCCS).

The post holder will lead on ensuring these services meet the needs of the local population, are accessible and have clear patient pathways and processes.

You will work closely with and support the Head of Specialist Community Nursing, actively contributing to the Patient Services management team, and the Senior Leadership team, taking individual and corporate responsibility for effectiveness and development.
